The Patient Care Coordinator serves as the central point of contact for patients within the medical facility, ensuring seamless and compassionate care from initial consultation through follow-up. The role involves coordinating appointments, managing communication between patients and clinical teams, facilitating referrals, and providing guidance on treatment plans and services. The Coordinator plays a vital role in enhancing the patient experience and supporting clinical efficiency.
Key Responsibilities:

Serve as the primary liaison between patients, families, physicians, and support staff.

Coordinate and schedule patient appointments, diagnostic tests, referrals, and follow-up visits.

Educate patients on procedures, treatments, and care plans, ensuring they understand the instructions and expectations clearly.

Assist patients with navigating the healthcare system, including insurance coverage, authorisations, and billing queries.

Maintain accurate and confidential patient records in compliance with legal and institutional standards.

Monitor patient satisfaction and respond to patient concerns or complaints with professionalism and empathy.

Collaborate with clinical and administrative teams to ensure timely delivery of care.

Track patient progress, communicate changes in status, and support transitions of care (e.g., from inpatient to outpatient services).

Qualifications & Requirements:
Education:

Associate’s or Bachelor’s Degree in Health Administration, Nursing, Social Work, or related field required.

Additional certification in Patient Care Coordination or Case Management is an asset.
Experience:

Minimum of 2 years in a healthcare setting, preferably in a coordination, case management, or administrative role.

Experience with Electronic Medical Records (EMR) systems preferred
