To facilitate the Admission process, please provide the following documents for both the Patient and the Next of Kin when booking your appointment with the Admissions officer:

• Two forms of Identification (options)
  • Barbados/Trident ID card
  • Valid Driver’s License
  • Valid Passport
  • National Registration/Social Security card
• Proof of current address
  • Utility bill (no older than three months) or,
  • Recent bank/credit union statement (only address header required)

• Insurance Card (Patient only – if applicable)
